Introduction:

In this article, you will learn how to add consent options to your forms with LeadsHook, ensuring GDPR compliance by obtaining explicit consent from your leads. There are two consent capture methods to choose from: Radio buttons and Checkboxes.

Option 1: Radio – offers a choice of 2 or more options where your leads can select any ONE option

How to Add Consent to Your Form

Step 1: Create Custom Field

  1. Go to Admin

  2. Click on “Custom Fields.”

  3. Select “Form”

  4. Press “Add” to create a new custom field.

Option 1: Create Radio Type Custom Field

  1. Enter a custom field

  2. Enter custom field name

  3. Set the type as “radio.”

  4. Enter values and labels for all choices, where the value will be saved as a custom field value and the label will be displayed on the form.

  5. Click “Add” for each Value-Label combination

  6. Select a default option (optional).  For GDPR all choice needs to be explicit

  7. For Radio button, select Required: Yes.  Because yes and no are explicit choices

  8. Set Visible: Yes to display on form.

Option 2: Create Checkbox Type Custom Field

  1. Enter a custom field

  2. Provide a custom field name.

  3. Set the type as “checkbox.”

  4. Enter values and labels each choice you want to offer.  Value  is what is saved into a custom field and Label is what is displayed on the form.

  5. Click “Add” for each Value-Label combination

  6. Select a default option (optional).  For GDPR all choice needs to be explicit

  7. For checkbox option, select Required: No.  Because choice is not explicit – users can choose none or any number of choices.

  8. Set Visible: Yes to display on form.

Step 3: Add Consent to Form

The Form User Interface has the following consent options

A text editors to add content BEFORE and AFTER your consent options.

And consent option which provides access to custom fields created in STEPs 2A and 2B.

View when after expanding consent options

Select the Checkbox or Radio custom field that you created.

Step 4: Publish the Decision tree and test.

Note:
With consent options incorporated, test your form to ensure that they function correctly and comply with GDPR requirements.
